KEENE, N.H. (MyKeeneNow) Nancy Folsom, a hospice RN case manager at Home Healthcare, Hospice & Community Services (HCS) in Keene, has been named the 2024 Caregiver Superstar of the Year by the Phaneuf Family Foundation. This recognition celebrates Folsom’s extraordinary dedication to her patients over her 21-year career, highlighting her holistic approach to end-of-life care.

Her nomination, submitted by Vanessa Sheehan, associate nursing director at HCS, emphasized her unique ability to connect with patients on a personal level in a news release.

Folsom also used the occasion to give back, making a donation to Amazing Grace Animal Sanctuary in Sullivan on Giving Tuesday.

The Phaneuf Family Foundation, established in 2019, assists families in New Hampshire and Vermont with funeral costs. Since its inception, the foundation has supported over 175 families, continuing its mission to provide critical end-of-life support across the region.
